The local police are willing to take my fingerprints, but do not have the fingerprint form. Where can I obtain this?
FBI Fingerprint Cards are used by most USA states and other foreign jurisdictions that do not have unique fingerprint cards. You can print the cards from this site:

Where can I get my fingerprints done?
In most jurisdictions, the local police or private fingerprinting company are trained in fingerprint rolling and provide this service for immigration purposes. In Canada, private fingerprinting services in larger centres may provide “livescan” prints that are sent electronically to the RCMP, allowing for much faster record production.
